29 CFR 1926.50(g): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) On or about February 7, 2013, and at times prior, employees were exposed to burns to the eyes and skin from anhydrous ammonia while cutting refrigeration system piping without the use of an eyewash station in the immediate area.

On or about February 7, 2013, an employee sawed into a live line of ammonia and created an ammonia leak.

29 CFR 1910.119(h)(3)(iii): The contract employer did not document that each contract employee had received and understood the training required by this paragraph. The contract employer did not prepare a record which contained the identity of the contract employee, the date of training, and the means used to verify that the employee understood the training: a) On or about February 7, 2013, and at times prior, the employer did not provide adequate training to ensure that employees cutting and opening refrigeration process piping containing anhydrous ammonia understood how to perform the work safely. On or about February 7, 2013, an employee sawed into a live line of ammonia and created an ammonia leak. The employer did not have a record which showed that the employee had received training to perform lockout tagout or open process piping.
