29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about March 18, 2013, the rotating chuck on the Porter Cable drill press (Serial Number 036216) was not guarded. Employee (s) exposed to the hazard of being caught in the rotating drill.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all rotating parts on machines are guarded.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:  On or about March 18, 2013, the grinder area, the outer sheath on flexible conduit on the Nelmor grinder was pulled apart exposing internal conductors. Employee(s) exposed to a potential electric shock when using the grinding machine.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the integrity of the outer sheathing of flexible conduit is intact and not exposing internal conductors.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:   On or about March 18, 2013, the disconnect panel boxes in the materials handling room were not readily accessible, exposing employees to an electrical shock hazard.     a) On or about March 18, 2013, the disconnect panel box by the silo machine was not accessible.    b) On or about March 18, 2013, the disconnect panel box by the Novatec vacuum pump #1, was not accessible.    c) On or about March 18, 2013, the disconnect panel box by the Novatec vacuum pump #2, was not accessible.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all electrical disconnects are provided sufficient access for operation or maintenance.
