29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe-boards: Note: Toe-boards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. (a) Mezzanine - Employees loaded skids on the mezzanine using a "Yale" cherry picker. They swung open an 8 foot, 8 inch section of guard rail and positioned the skid of the cherry picker on the floor of the mezzanine. The employees were exposed to a fall of 10 feet 6 inches from the mezzanine to the floor below when they disconnected their personal fall arrest harness from the anchor point on the cherry picker and dismounted the machine to work at the mezzanine level with the gate open and the leading edge unprotected on or about March 20, 2013.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a) Office - The "Panasonic" NR-65GS mini fridge in the office did not have a continuous path to ground, office employees used the mini fridge for drinks, snacks and lunches and were exposed to electric shock hazards on or about, March 20, 2013.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage. (a) Syrup Production Area - Employees operated syrup transfer pump to transfer syrup from production tanks to the assembly line. The flexible cord for the pump's "Reliance Electric" three phase 220 volt electric motor had damage to the insulation wich penetrated to the internal conductors, the damaged area was covered with electrical tape and employees were exposed to electrical shock hazards during production on or about March 15, 2013.
