Jobsite:  Harrison Power Station, RR 20, Haywood, WV  26366 - Respondent Burnham Industrial Contractors, Inc., exposed employees to the recognized hazard of stepping onto and falling partially or completely through the thin inner lagging of an operational process ventilation duct thereby exposing them to potential lacerations, thermal burns, and asphyxiation from the dangerous environment inside the duct.

Employees were exposed to hazards including severe metal lacerations, thermal burns, and an oxygen deficient atmosphere inside the duct as determined on March 21, 2013.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED.
