29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about March 19, 2013, at Hwy 1177 FM 2376, Aledo, TX employees installing concrete drainage pipe at the west end of an unprotected vertical wall of a trench excavation were not protected from cave-in hazards.
