29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (a) Location: 4 Cameron Avenue, Somerville MA 02143 On or about 03/20/2013, a competent person did not perform a frequent and regular inspection of the jobsite to identify safety and health hazards such as: The power cord on the space heater having the outer insulation pulled away, missing ground prongs on numerous electrical devices, unprotected temporary lamps, an energized electrical panel with exposed live parts, no GFCI protection throughout site, energized electrical outlets missing faceplates, numerous damaged electrical cords in use and a damaged step ladder in use.

All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection. Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.
