29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a)	Summit Assembly Inc. DBA Summit Integrated Systems, 9447 Niwot Rd, Niwot, CO 80504: On and before March 19, 2013, the employer did not ensure that employees working at a height of approximately 30 feet were provided with an adequate personal fall arrest system. 29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee:    (a)	Summit Assembly Inc. DBA Summit Integrated Systems, 9447 Niwot Rd, Niwot, CO 80504: On and before March 19, 2013, the employer did not ensure that employees were using anchorages rated to support 5,000 pounds. Employees were using weight distribution wires that were not rated per the standard for anchorages    (b)	Summit Assembly Inc. DBA Summit Integrated Systems, 9447 Niwot Rd, Niwot, CO 80504: On and before March 19, 2013, the employer did not ensure that employees were using anchorages rated to support 5,000 pounds. Employees were using a guardrail that was not rated per the standard for an anchorage.
