29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, a safety net system, or a personal fall arrest system. a) Job site, 269 Lebanon Avenue, Pittsburgh, Pennsylvania, 15228: Observed on or about May 20, 2013, employees applying siding to the second floor of a residential dwelling were not protected from falling. The employees were standing on roof with a slope of 3 in 12 (vertical to horizontal). The employees were exposed to a fall hazard of approximately 18 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Job site, 269 Lebanon Avenue, Pittsburgh, Pennsylvania, 15228: Observed on or about May 20, 2013, employees applying siding to the second floor of a residential dwelling were not protected from falling. The employer did not provide a fall hazard training program for all employees. The employees working on the roof were exposed to a fall hazard of approximately 18 feet to the ground below.
