29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    a. In the loading area of the processing building, where product was loaded onto flatbed trucks without railing or other fall protection (truck beds 55-57 inches on two trucks in the building at the time of the inspection), employees loading and securing the load while standing on the truck bed and at somewhat higher heights when standing on the loaded product, were exposed to a fall.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

Where 5 gal air tanks were used for maintenance in the truck building, safety relief valves were not regularly tested. 29 CFR 1910.147(c)(4)(ii):     Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):    a. Specific lockout procedures for each type of equipment where specific procedures were required, were not developed for all equipment, such as, but not limited to, the Putty Machine, Rex Machine, Plug Cutter, and Chop/Defect Saws (note: lockout was being performed and training had been provided).
