Adjacent to the Entrance to the Cabell Huntington Children's Hospital: An employee operated the SkyTrak 6036 from its location on site to the trailer, loaded it on the trailer and unloaded it from the trailer after repairs and parked it back on site had not completed the hands on and the practical training with regard to rough terrain forklifts as determined on 3-27-13. ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM

Adjacent to the Entrance to Children's Hospital: The employee unloading the SkyTrak 6036 rough terrain fork lift off the trailer and then operating it to where it was parked at the site was not wearing a seat belt due to the forklift was not equipped with a seat belt as determined on 3-20-13.
