29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On March 19, 2013 an employe was not protected from falling through a floor hole, exposing the employee to a fall hazard of approximately 10 feet. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
