29 CFR 1910.178(l)(1)(i): The employer did not provide training and evaluation for powered industrial truck operators: At the time of inspection, operators were not trained in the safe use of powered industrial trucks. Training shall consist of a combination of formal instruction (e.g., lecture, interactive computer learning, video tape, written maerial), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): At the time of inspection the Hyster Lift Truck (Serial Number D004V03304K) was in need of repair. The steering was not in a safe operating condition. The steering wheel was easily removable from the steering column. The steering column itself was not attatched properly. Additionally, the clutch brake was help up using a bungee cord.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) At the time of inspection, the light shields on the Herr-Voss Stam Co. cut-to-length line were not operating as intended. Employees were able to access inside the final cut area while the machine was in operation. The employees were exposed to the moving machine parts and ingoing nip points of the material lifting device. B) At the time of inspection, perimeter guarding was missing or not present on all areas of the Herr-Voss Stam Co. cut-to-length line. Perimeter guarding was missing next to the access stairs allowing employees to access the area where coiled steel is picked up my automatic machinery.
