29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    (a) On or about March 20, 2013 - Manufacturing area, an employee operating a grinder was exposed to continuous noise at 104.2 % of the allowable 8-hour time-weighted average sound level of 90 dBA, or an equivalent sound level of approximately 90 dBA.  The sampling was performed for 420 minutes during the day shift on 3/20/13.  Zero exposure was assumed for the unsampled period of time of 60 minutes.
