29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about March 19, 2013, on the west side of the work site, employees engaged in cornice activities utilized 2 extension cords that were missing the ground pins to operate Makita skill saws exposing employees to electrical hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about March 19, 2013, on the west side of the work site, an employee was engaged in cornice activities on a residential building from a single pole scaffold without fall protection. The employee was exposed to the hazard of falling approximately 14 feet to the ground below.
