29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Site - 10673 Cross Creek Blvd. Tampa, Fl. 33647 Northeast Roof Area - Second Skylight Area A 4'x4' section of plywood covering a skylight hole was removed allowing for a 20' foot fall onto a pallet of drywall mortar bags on or about, 03/15/13. OR IN THE ALTERNATE: 29 CFR 1926.502(d)(16)(iii) : The personal fall arrest system was not rigged that an employee can neither free fall more than 6 feet, nor contact any lower level: a) Site - 10673 Cross Creek Blvd. Tampa, Fl. 33647 Northeast Roof Area -Second Skylight Area The fall arrest system was not rigged as required and allowed for a 20' foot fall onto a pallet of drywall mortar bags upon stepping into an unguarded skylight hole, on or about, 03/15/13.
