Loaction: 6760 Belkenton Ave, Cincinnati, OH a)On or about 20 March, 2013, the employer did inot have a competent person inspect the job site and in doing so exposed employees fall hazards while working on the roof and ceiling and exposed unprotected employees to lead and asbestos.

29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work had the strength and structural integrity to support employees safely: Location: 6760 Belkentom Ave, Cincinnati, OH a) On or about 20 March 2013, the employer allowed employees to work in a crawl space that had not been assessed to determine its abilty to support the weight of the employees. An employee was working in the space above the second floor ceiling had a leg pass though the ceiling.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: 6760 Belkenton Ave, Cincinnati, Oh On or about 20 March, 2013, employees were working on the roof of a three story structure with no fall protection in place. Employees were repairing roof drains and working next to the roof edge. There was no gaurdrail in place and employees wre not weraing personal fall arrest systems.
