29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to the items of this section: 3550 Keystone Dr., Omaha, NE 68134 (Site-Wide): The employer did not ensure that the lockout procedures used by employees to service equipment were adequately specific. The procedures had specific steps for shutting the equipment down and for starting the equipment up for production purposes, but did not have procedures for doing so for servicing equipment, including, but not limited to, the 500-ton Injection Molder (JSW-3). Abatement certification and abatement documentation is required for this violation. The abatement certification sheet is enclosed.
