29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,  a. South Wall/Red Bud Industries Steel Coil Machine Line # 1/Bay # 2 - machine operators were required to use a stair  platform 4.5 feet high without a midrail. Employees were thereby exposed to a fall hazard.

29 CFR 1910.26(c)(2)(iv):   Care of Ladders - Ladders must be maintained in good usable condition at all times.   a. Bay # 2/ Red Bud Industries Steel Coil Machines/Line # 1 Area - On or about March 22, 2013, a portable metal ladder was observed damaged. Employees were thereby exposed to falls. The conditions of the portable metal ladder were the following;   1. The right front metal leg piece was broken and bent at the horizontal welded square tubular steel frame.   2. The left front metal leg piece was broken and bent at the horizontal welded square tubular steel frame.   3. The rubber coaster feet on both front legs were missing from their initial intended design.

29 CFR 1910.212(a)(1):   Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a. Bay # 2/Coil Processing Equipment/Northeast Area - Red Bud Line # 2 - On or about March 22, 2013, employees were required to access the unguarded back opening of the Red Bud Industries # 2 - coil processing machine to manually remove the steel coil scrap cart. Employees were thereby exposed to the hazards of contact with rotating parts and sharp steel/metal pieces.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition shall be removed from service. All repairs shall be made by authorized personnel.  a. Production/Storage Warehouse Areas - On or about March 28, 2013, the employer's regular maintenance program for  the propane powered industrial trucks did not include an analysis of exhaust gasses that included carbon monoxide. The following propane powered industrial trucks were found to be in unsafe operating conditions;   1.  The Cat propane forklift truck (T 1500 with Serial # 5MBO1745) had a carbon monoxide level of 4.4 % while idling.   2. The Cat propane forklift truck (T 1500 with Serial # 5MBO1745) had a carbon monoxide level of 1.8 % at full throttle.

29 CFR 1910.178(q)(7):   Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  a. Production/Warehouse Storage Areas - On or about March 22, 2013, employees were required to operate propane powered industrial trucks and the daily inspections records were not showing any defects and/or corrections made. Among the unsafe conditions founds were the following;  1. The industrial forklift truck - Yale #2 (Serial # B81600023411 and Model # GLCO80LGNGAV035) had its right front head light off.  2. The industrial forklift truck - Yale # 1 (Serial # B818DO3526W and Model # GLCO80LGNGAV035) had a non-audioable  horn.

29 CFR 1910.305(g)(2)(iii):   Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.   a. Dock Area/Bay # 4/Right side  - On or about March 22, 2013, the power cord on the electrical fan had its outer insulation pulled away from its fittings. Employees were thereby exposed to electrical hazards at the following locations;    1. The outer insulation on the electrical cord was pulled away from its plug's fitting.   2. The outer insulation on the electrical cord was pulled away from the back of the motor's fitting.

29 CFR 1910.22(b)(1):   Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repairs, with no obstruction across or in aisles that could create a hazard.  a. Receiving Dock/Bay # 5 /Coil Storage Area - On or about March 22, 2013, employees were required to conduct warehouse operations without adequate clearance between passageways and aisles. Employees were thereby exposed to struck-by hazards in the listed areas;   1. The narrow aisle between the shipping dock/bay # 4 and the metal column of the old building showed dents and missing aluminum pieces.  2. The narrow aisle between the stored steel coils and the concrete bases of the building structure columns at the warehouse behind the red bud industries steel coil machine line # 1 showed scrapes and missing paint on the concrete.
