29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) Xtreme Construction, LLC @ 226 Pearl St., Denver, CO 80203: On 3/20/13, the exposing employer did not ensure that each employee engaged in roofing operations were trained to recognize hazards of working at elevation and trained in the use of fall protection. Three employees worked on a 7:12 pitch, single family, residential roof. This condition exposed employees to fall hazards of approximately 12.5 ft.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented: (a) Xtreme Construction, LLC @ 226 Pearl St., Denver, CO 80203: On 3/20/13, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in roofing work. Four employees worked on a 7:12 pitch, single family, residential roof. This condition exposed employees to fall hazards of approximately 12.5 ft. Xtreme Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number, 514338, citation number 1, item number 2 and was affirmed as a final order on August 14, 2012, with respect to a workplace located at 652 Dakota Way, Windsor, CO 80550.
