29 CFR 1926.1053(a)(3)(i): Rungs, cleats, and steps of portable ladders (except as provided below) and fixed ladders (including individual-rung/step ladders) shall be spaced not less than 10 inches (25 cm) apart, nor more than 14 inches (36 cm) apart, as measured between center lines of the rungs, cleats and steps. (a) McClone Construction, 3100 Pearl Street, Boulder, CO 80108: On and before March 14, 2013 the exposing employer did not ensure that the rungs of a homemade wooden ladder were spaced safely apart in that the distance between rungs was 15.5'' and the distance across rungs was 19''. This condition exposed employees to fall hazards.

29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. (a) Reylenn Construction, 3100 Pearl Street, Boulder, CO 80108: On and before March 14, 2013 the exposing employer did not conduct frequent or regular inspections of materials by a competent person in that rebar was bent down and protruding from the garage floor in building B to allow forklifts to drive over it. This condition exposed employees and subcontractor employees to trip, cut and puncture hazards.
