None of the four wheels on the scaffold was locked.   b) On or about 03/21/13, on construction site inside former bank building, 1308 Niagara Falls Blvd., Tonawanda, NY: Employee was exposed to potential fall of approx. 5 ft.11 in. while removing overhead ductwork, while on the platform of a narrow Bil-Jax mobile metal frame scaffold. None of the four wheels on the scaffold was locked.   NO ABATEMENT CERTIFICATION REQUIRED
