29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On or about March 18, 2013, the employer did not conduct frequent inspections and ensure that the employees performing construction activities were protected from fall hazards.     a) Employees performing masonry activities worked from a tubular welded frame scaffold with a number of deficiencies that created an unsafe working condition.  Employees were allowed to work on a tubular welded frame scaffold without toe boards and guardrails on all open sides.  Employees were exposed to fall and struck by hazards.    b) Employees performing framing activities on the front side of building on a 10 inch platform were exposed to a fall hazard of approximatley 15 feet to the ground below.   c)  Employees were allowed to work with equipment such as the Rigid air compressor and a concrete mixer whose belts and pulleys were not guarded.  Employees were exposed to flying chip hazards.
