29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): At this site, the employer did not post critical safety instructions in lieu of written operating procedures or job safety analysis (JSA) at process equipment where non-routine tasks were performed and wherein the release of highly flammable chemicals (natual gas liquids) was possible. These operations tasks included but were not limited to, the maintaining and flushing of level sight glasses. Or in the alternative: 29 CFR 1910.1200(e)(1)(ii): The written hazard communication program did not include the methods the employer used to inform employees of the hazards of non-routine tasks (e.g. the cleaning of reactor vessels), and the hazards associated with chemicals contained in unlabeled pipes in their work areas:
