29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard. Employees engaged in material handling activities throughout the facility were exposed to fall hazards of approximately 49 and 50 inches to the concrete below in that mid-rails were not provided at the following locations: a) Loading bay #26: The fall hazard was found to be approximately 50 inches at the front of the dock plate to the concrete below. b) Loading bay #22: The fall hazard was found to be approximately 49 inches at the sides of the dock plate to the concrete below. c) Loading bay #31: The fall hazard was found to be approximately 49 inches at the sides of the dock plate to the concrete below. d) Loading bay #90: The fall hazard was found to be approximately 49 inches at the sides of the dock plate to the concrete below.

29 CFR 1910.303(f)(2): Services, feeders, and branch circuits. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. Employees engaged in material handling activities throughout the facility were exposed to electrical shock hazards in that each disconnect in breaker panel T section 2 was not clearly labeled with their purpose and the purpose of each breaker was not evident by location of the disconnect.
