29 CFR 1926.453(a)(2):     Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANS.    Aerial lifts may be "field modified" for uses other than those intended by the manufacturer provided the manufacturer has certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969 and this section and to be at least as safe as the equipment was before modification.     Workers were engaged in commercial construction activities replacing existing skylights and framing parts. Workers utilized a JLG 600 S articulating, extensible, boom aerial lift, with wooden 2 inch x 4 inch brackets fastened to the basket, to hoist 6.0 x6.0 foot glass panels to the upper roof area. Workers were exposed to potential fall hazards in that the wooden brackets did not conform to ANSI A92-1969 requirements.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      Workers were engaged in commercial construction activities replacing existing skylights and framing parts. Workers on the roof area were exposed to potential 20.7 foot fall hazards in that the workers did not utilize a proper fall protection system.
