29 CFR 1910.23(d)(1)(iii):     On stairways less than 44 inches wide having both sides open, there was not one stair railing on each side.    On 3/21/2013, employees working at Monroe City Care Center, Inc. located at 1010 Highway 24 & 36 East, Monroe City, MO 63456, were exposed to a fall hazard.  Specifically, the railing for the stairway leading to the storage loft in the garage was missing a railing.

29 CFR 1910.23(e)(1):     A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail was not smooth-surfaced throughout the length of the railing. The intermediate rail was not approximately halfway between the top rail and the floor, platform, runway, or ramp.     On 3/21/2013, employees working at Monroe City Care Center, Inc. located at 1010 Highway 24 & 36 East, Monroe City, MO 63456, were exposed to a fall hazard.  Specifically, the storage loft in north side of the garage was missing a railing on the east side of the stairway.  Additionally, the railing on the west side of the stairway was 36" high and missing an intermediate rail.

On 3/21/2013, the exit sign above the door leading out of the west side of Monroe City Care Center, Inc. located at 1010 Highway 24 & 36 East, Monroe City, MO 63456 was not illuminated.

29 CFR 1910.132(e):     Defective and damaged equipment. Defective or damaged personal protective equipment was being used.    Employees working at Monroe City Care Center, Inc. located at 1010 Highway 24 & 36 East, Monroe City, MO 63456 used defective and damaged personal protective equipment.  Specifically, the employees working in the kitchen used a face shield with a broken strap while cleaning and employees working in the soiled utility room and laundry used damaged goggles while rinsing soiled linen and changing chemicals; the goggles were severely scratched and scuffed, which impacted the employees ability to see through them.

29 CFR 1910.334(a)(2)(ii):     Portable electric equipment:  There was a defect or evidence of damage that exposed an employee to injury.  And, the defective or damaged item was not removed from service.  Employees were using it without repairs and tests needed to render the equipment safe prior to use.    Employees working at Monroe City Care Center, Inc. located at 1010 Highway 24 & 36 East, Monroe City, MO 63456 use  a defective and damaged portable 3  HP air compressor, serial # 105619. The compressor is located in the garage and used to service equipment as needed. The compressor has had a household light switch wired-in to bypass a defective high pressure cut-out switch.  The switch does not have a continuous path to ground; there is no ground wire attached to the exposed metal switch parts (only hot wires wired to the switch) and has exposed neutral wires near the switch.
