29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section.    The employer does not protect employees from falling when working 15 feet or more above lower levels.  This violation was observed on or about March 20, 2013, on the south side of a steel erected building where employees were exposed to an approximate 16 foot fall hazard while connecting steel beams without the ability to be tied off.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the emploer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls while working 15 feet or more above a lower level with an unprotected side or edge.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.    The employer does not extend the ladder 3 feet above the upper landing of steel structure.  This violation was observed on or about March 20, 2013, on the south side of a steel structure where employees were exposed to a fall hazards when climbing a ladder that did not extend its side rails at least three feet above the steel beam to access the structure.     Pursuant to 29 C.F.R. 1903.19, within 10 (ten) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees extend ladder side rails at least 3 feet above upper landing surface to gain access.
