29 CFR 1910.179(j)(2): The following items were not being inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections:       On or about March 20, 2013, and at times prior thereto, overhead cranes being used to lift heavy objects were not inspected monthly.  Employees were exposed to dropped-on or struck-by hazards from suspended loads.

29 CFR 1910.303(c)(3)(i): Conductors were not spliced or joined with splicing devices identified for the use or by brazing, welding, or soldering with a fusible metal or alloy.   soldered splices were not spliced or joined mechanically and electrically secured first without solder and then soldered.  All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose.         On or about March 20, 2013, in the east end of Bay 5 a damaged extension cord was observed not spliced properly being used to provide power to equipment.  This exposed to electrical hazards.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    On or about March 20, 2013, and at times prior thereto, at the facility in three incidents the path to ground from circuits, equipment, and enclosure was found not permanent and continuous:     Incident 1: In the east end of Bay 5 areas an extension cord was observed having ground pin missing being used to provide power to other equipment.    Incident 2: In the east end of Bay 4 areas a power cord of a fan was observed having missing ground pin being used.     Incident 3: In the east end of Bay 4 areas a four-plex outlet device was observed having the path to ground not permanent and continuous.    Employees were exposed to electrical hazards.

29 CFR 1910.304(b)(2): Outlet devices had an ampere rating less than the load being served:      On or about March 20, 2013, or at times prior thereto, in the NE Bay 4, South Bay 7 and Shipping/Receiving areas outlet devices were observed having electrical equipment plugged into the outlet device which had a current rating less than the load being served.  This exposed employees to fire hazards.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l)    On or about March 20, 2013, and at times prior thereto, in the shipping/receiving areas at the establishment the employer allowed employees to operate forklift trucks without prior training or evaluation provided by a qualified person.  Employees were exposed to hazards caused by inexperienced forklift truck operators.
