29 CFR 1926.303(c)(5):     When safety guards are required, they shall be so mounted as to maintain proper alignment with the wheel, and the guard and its fastenings shall be of sufficient strength to retain fragments of the wheel in case of accidental breakage. The maximum angular exposure of the grinding wheel periphery and sides shall not exceed 180 deg.    a. On the west side of the house where employees were cutting stone, the guard was missing from the angle grinder, thereby exposing employees to a struck-by hazard.

29 CFR 1926.405(a)(2)(ii)(J):     Extension cord sets used with portable electric tools and appliances shall be of three-wire type and shall be designed for hard or extra-hard usage. Flexible cords used with temporary and portable lights shall be designed for hard or extra-hard usage.  NOTE: The National Electrical Code, ANSI/NFPA 70, in Article 400, Table 400-4, lists various types of flexible cords, some of which are noted as being designed for hard or extra-hard usage. Examples of these types of flexible cords include hard service cord (types S, ST, SO, STO) and junior hard service cord (types SJ, SJO, SJT, SJTO).    a. On the west side of the house where employees were cutting stone, a damaged extension cord that was not rated for hard or extra hard use was being used, thereby exposing employees to a contact hazard.

29 CFR 1926.451(b)(1)(i):     Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:    Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) shall be installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform).    a. On the third bunk of the tubular welded frame scaffold where employees were installing stone to the front of the house, employees were working on a singular 2 inch by 10 inch by 10 foot plank, thereby exposing employees to a fall hazard.

29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    a. On the west side of the house where employees were working from the third bunk of a tubular frame scaffold, crossbraces were used as a means of accessing the scaffold, thereby exposing employees to a fall hazard.

29 CFR 1926.451(g)(4)(i):     Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.    a. On the third bunk of a scaffold where employees were installing stone to the front of the house, guardrails were not installed, thereby exposing employees to a 14 foot fall hazard.
