29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: At the time of the inspection, two Plasterers were working on the West roof of the Westwood Baptist Church and used a horizontal lifeline that was not approved. The lifeline was made of several types of retractable lifelines connected to each other in a manner they were not designed to be used.
