29 CFR 1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  1) At the TEXWRAP machine (Model 2219, SN #D1164), the drop cord from the ceiling junction box was plugged into an extension cord which ran along the drop ceiling into another extension cord servicing both the electrical box for the TEXWRAP machine and the in feed conveyor for the TEXWRAP machine.  Extension cords were not used according to the manufacturer's instructions.    2) At the portable conveyor dedicated to the Shrink Wrap machine work area, the drop cord was plugged into an extension cord, pinched between the conveyor belt and table, and traveled across the floor by employees feet into another extension cord which serviced the portable conveyor.   Extension cords were not used according to the manufacturer's instructions.      IN THE ALTERNATIVE:    29 CFR 1910.305(g)(1)(iv)(A)  Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  1) At the TEXWRAP machine (Model 2219, SN #D1164), the drop cord from the ceiling junction box was plugged into an extension cord which ran along the drop ceiling into another extension cord servicing both the electrical box for the TEXWRAP machine and the in feed conveyor for the TEXWRAP machine.  Temporary wiring was used in lieu of fixed wiring.    2) At the portable conveyor dedicated to the Shrink Wrap machine work area, the drop cord was plugged into an extension cord, pinched between the conveyor belt and table, and traveled across the floor by employees feet into another extension cord which serviced the portable conveyor.   Temporary wiring was used in lieu of fixed wiring.

29 CFR 1910.305(a)(2)(x)  Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:    1) At the portable conveyor dedicated to the Shrink Wrap machine work area, the drop cord was plugged into an extension cord, which was pinched between the conveyor belt and table, and traveled across the floor by employees feet into another extension cord that serviced the portable conveyor.   The extension cord was not protected from the pinch point.

29 CFR 1910.305(g)(2)(iii)  Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  At the CONFLEX in feed conveyor (SN # CJ08140-01) the drop cord was plugged into an extension cord which serviced the in feed conveyor and the cord and plug lacked strain relief.
