Production area- On or about March 21, 2013, the Edward Pearson Press Brake was not equipped with a guard to cover the foot control which actuates the press brake, as prescribed by the American National Standards Institute (ANSI) B11.3-2002.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).     a. On or about March 21, 2013, in the work area, employees were required to operate several styles of powered industrial vehicles. The employees did not receive training on the safe operation of the powered industrial vehicles as required by 29 CFR 1910.178.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.   a. Press Brake Area - On or about March 21, 2013, employees were required to operate the Accupress and Edward Pearson hydraulic press brakes on a daily basis. The employer failed to provide guarding for the Accupress and Edward Pearson press brakes. Employees were thereby exposed to the hazards associated with unguarded points of operation during the operating cycles.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service:   The employer does not protect each employee by ensuring industrial trucks are examined before being placed in service.     a)   This most recently occurred on March 21, 2013, the employer does not require on a daily basis the pre-operation inspection of fork trucks and standup vehicles prior to their use.    To abate this hazard in the future, the employer must ensure industrial trucks are examined before being placed in service.   This employer has been cited for this practice that included: Hoist Liftruck Mfg, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(7) which was contained in OSHA inspection number 400064, citation 1, item 2b, issued on September 28, 2012, and became final order on about 10, 23, 2012, with respect to a workplace located at 6499 W. 65th Street, Bedford Park, IL 60638.     Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how industrial trucks will be examined before being placed in service.

The employer does not protect each employee by inspecting cranes for defects at intervals defined in paragraph (j)(1)(ii) of this section.   a)  This most recently occurred on February 28, 2013, the employer does not conduct "frequent inspections" of its jib, trolley and bridge cranes to determine safety hazards.   To abate this hazard in the future, the employer must ensure cranes are inspected for defects at intervals defined in paragraph (j)(1)(ii) of this section.     This employer has been cited for this practice that included: Hoist Liftruck Mfg, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.179(j)(2) which was contained in OSHA inspection number 400064, citation 1, item 3a, issued on September 28, 2012, and became final order on about October 23, 2012, with respect to a workplace located at 6499 W. 65th Street, Bedford Park, IL 60638.
