29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Grassy Area 500 Block of Hanover Village - (a) An employee performed repair work on a sewer pipe in a 10- foot deep, 4' by 8' trench and did not have a stairway, ladder, ramp or other safe means in place to provide a safe means of egress. THe employee used the bucket on the back hoe as access and egress, observed on or about 3-22-2013. Abatement certification required.

29 CFR 1926.652(a)(1): 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Grassy Area 500 Block Hanover Village - An employee performed repair work on a sewer pipe in a 10- foot deep, 4' by 8' trench in type B soil and used 2x4 pine boards and 1/2 inch particle board sheets as shoring. This system did not constitute an adequate system as outlined in 29 CFR 1926.652(c) and exposed the employee to a serious hazard, observed on or about 3-22-2013. Abatement documentation required.
