29 CFR 1910.1030(f)(2)(iv): The employer did not ensure that employees who declined to accept the Hepatitis B vaccination offered by the employer signed the statement in appendix A:     a) On or about March 21, 2103 and at times prior, the employer's records for employees with occupational exposure who did not proceed with the Hepatitis B vaccinations were incomplete as it relates the their declinations statement:   Employee #1 was hired on February 28, 2103 as a Certified Nursing Aid (CNA) and the employee's Hepatitis B Vaccine Administration Record (no form #) did not indicate if the Hepatitis B vaccinations were requested or were declined by the employee.  The form only had a block that addressed titer acceptance/declination and the form was not signed.     Employee #2 was hired on March 1, 2013 as a CNA and the Hepatitis B Vaccination Administration Record (FRM 56103-01) did not indicate if the Hepatitis B vaccinations were requested or declined. The employee only signed on that portion of the form that discusses the titer explaination.   Employee #3 was hired on March 8, 2013 in dietary and the Hepatitis B Vaccination Administration Record (FRM 56103-01) did not indicate if the Hepatitis B vaccinations were requested or declined. The employee only signed on that portion of the form that discusses the titer explaination.   Employee #4 was hired as a dietary aid and the Hepatitis B Vaccination Administration Record (FRM 56103-01) did not indicate if the Hepatitis B vaccinations were requested or declined.  The only notation on the form is that a titer test was due in August and that on September 12, it still was not done. 29 CFR 1910.1030(h)(5)(i): The employer did not establish and maintain a sharps log for the recording of percutaneous injuries from contaminated sharps:       a) On or about March 21, 2013 and at times prior,  the employer did not adequately maintained the sharps injury log to cover the calendar year 2009 in that the required documentation of the one incident was inadequate. Missing information on the log included: time of the incident, the location of the event and which finger was involved.       b)  On or about March 21, 2013 and at times prior,  the employer did not adequately maintained the sharps injury log to cover the calendar year 2009 in that the log with its one incident contained the identity of the employee thus not protecting the confidentiality of the injured employee.       On June 4, 2009, an identified employee received a needlestick on a finger on the left hand from a syringe used to administer medication.        c) On or about March 21, 2103 and at times prior, the employer had not maintained a sharps injury log to cover the calendar year 2012 in that there was a needle stick incident that was not recorded:        On April 4, 2012, an employee received a needle stick in the 2nd finger of the right hand when the needle was being removed from the pocket.  The needle was used to administer a insulin shot.               The Kindred Healthcare Inc dba Malley Healthcare and Rehabilitation Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 313417974, citation number 2, item number 2 and was affirmed as a final order on 5/10/2010, with respect to a workplace located at 401 Malley Drive Northglenn, CO      Abatement Note:  Abatement certification is required for this item (see enclosed "Sample Abatement-Certification
