29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about March 20, 2013 and at times prior, employees were exposed to fall hazards of approximately 9 feet high where they were walking up/down the sloped portion of the roof to access the flat portion of the roof. The employees were not protected with a fall protection system.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about March 20, 2013 and at times prior, an employee wearing a personal fall arrest system was exposed to fall hazards while working on a 9 feet high roof in that the employee was using a lifeline with excessive slack which would not limit him from falling less that 6 feet in the event of a fall.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) On or about March 20, 2013 and at times prior, employees were exposed to fall hazards of approximately 9 feet high where they were accessing a secondary roof by climbing over a parapet that was approximately 3 feet high. The access point from the parapet to the roof was not provided with a stairway or ladder.
