The employer did not ensure that the scrag mill supervisor required that employees performing lockout work followed the procedures, such as, but not limited to, after locking out equipment authorized employees keeping the key in their possession, and ensuring that employees were authorized to lockout and that locks were placed on the energy control device by the employee performing the work.
