29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a) Northside Crossing Apartments, Palymra Road - An employee was exposed to an 11 foot - 10 inch fall from the platform of a pump jack scaffold which had not been provided with midrails or endrails, on or about March 7, 2013. No abatement certification or documentation required.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Northside Crossing Apartments, Palmyra Road - Employees were not trained to recognize hazards in the safe use of a pump jack scaffold and how to correct hazards such as, but not limited to fall hazards, on or about March 7, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
