The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions: Building 1202 construction project, Presidio, CA - On or about March 21, 2013: An employee applying Ultrabond ECO 185 Flooring Adhesive was not wearing personal protective equipment including eye and hand protection.

29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced: Building 1202 construction project, Presidio, CA - On or about March 21, 2013, and at times prior thereto, employees were not trained on the hazards associated with Ultrabond ECO 185 Flooring Adhesive. Employees had not been trained on all chemical and materials used at the worksite including the hazards, exposures, PPE, labeling and other forms of warnings on worksite chemicals and materials including Ultrabond ECO 185 Flooring Adhesive.
