29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section:   a. At the machine room exit, a four step stairway outside the building was not provided with a handrail system.   b. At the roof area, a 23 step stairway leading from the maintenance roof top to the roof over the boilers was not provided with a midrail. The stairway had only a handrail installed at 29 inches from the nosing of the risers.    c. Glycol Chiller motor area,  a 6 step stairway leading to the motors was not provided with handrails.

29 CFR 1910.119(e)(3)(i): The Process Hazard Analysis did not address the hazards of the process:   a. The PHAs did not address the hazards of the process to include the following:    1.The ventilation system for the machine room was not designed to exhaust ammonia vapors out of the room and into a safe area.  The three wall mounted fans on the north wall were exhausting the vapors into a space between the buildings permitting vapors to travel into areas of the plant such as but not limited to the boiler room.   2. The machine room was not sealed properly in that; the north wall of the machine room did not extend to the ceiling.  The wall terminated approximately two to three feet below the room ceiling.  Additionally, openings where piping passed through the walls the opening(s) were not sealed.  In the event of a release refrigerant could escape into unventilated areas of the facility.

At the machine room, the working space in front of the breaker panel was used for storage. Tool boxes, poly drums containing a cleaner sanitizer and other equipment were stored right in front of power panel CP 3/4/7.    b. Boiler room east wall, the working space in front of 6 electrical panels was used for storage, in that, a poly drum and a boiler feed tank supplying a solution for alkaline control were stored in front of the panels.

Glycol cooling pump area, a red and a black extension cords were used to provide power to the air pumps.   b. Filler number 6 small bottle area, employees were exposed to electrical hazards, in that, two black extension cords were used to provide power to the sanitizer pumps.
