29 CFR 1910.178(l)(1)(i):     The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    Throughout the Company:  The employer failed to ensure that each operator of a powered industrial truck had been trained in the manner described in the standard.       Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.303(b)(1):     Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:       Worksite located at 2627 W. US HWY 30, Grand Island, NE:  An electrical box mounted an a carpet mule (carpet roll lifting device) was being secured using duct tape as opposed to the designed mount.  The duct tape was also securing the box cover in place.         This item was abated during the inspection.  No additional documentation is necessary.

29 CFR 1910.305(g)(2)(iii):     Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:     Worksite located at 2627 W. US HWY 30, Grand Island, NE; Show Room:  The cord for the Carpet Mule (carpet roll lifting apparatus) was pulled away from the male plug exposing the insulated conductors within as well as transferring the tension to the terminal screws within the plug.         This item was abated during the inspection.  No additional documentation is necessary.

29 CFR 1910.334(a)(3)(ii):     Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacle:    Worksite located at 2627 W. US HWY 30, Grand Island, NE; Show Room:  An extension cord used to power a carpet mule (carpet roll lifting device) as well as a carpet cutting table was missing the grounding conductor.        Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.
