Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by injuries from falling coils of steel.    a) R. K. Industries, Inc. - Ottawa, Ohio: On or about March 14, 2013, employees that assist with steel coil handling and/or operate the Regal Industries coil car (s/n 5782349), associated with the Ravne press, were exposed to hazards associated with falling coils.  The employer did not implement a risk assessment/risk reduction strategy for equipment modifications, develop safe operation procedures, train employees, or provide safeguarding to protect employees from hazards associated with the metal processing system.  An employee was injured when struck by a steel coil that fell from the coil car as it was being moved to the uncoiler.      b) R.K. Industries, Inc. - Ottawa, Ohio: On or about March 26, 2013, employees that assist with steel coil handling and/or operate the Dallas Industries coil car (s/n 22495) associated with the Aida press, were exposed to hazards associated with falling coils.  The employer did not develop safe operation procedures, train employees, or provide safeguarding to protect employees from hazards associated with the metal processing system.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.176(b): Storage of material created a hazard:    a) R. K. Industries, Inc. - Ottawa, Ohio: On or about March 14, 2013, the employer did not assure that coils of steel stored near the metal processing system associated with the 600 ton Ravne power press were secured from rolling.  Coils of steel, as wide as 24 inches and weighing up to 5,000 pounds each, were routinely stored adjacent to areas accessed by operators and fork trucks.
