29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a) On or about March 26, 2013; at the center of the jobsite, the employer allowed employees to utilize a welding electrode oven with an electrical plug that was missing a ground pin. The electrode oven was used to store welding electrodes at an ideal temperature to prevent moisture pick-up. The missing ground pin prevented the electrode oven from having a continuous path to ground.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. a) On or about March 26, 2013; at the center of the jobsite, the employer allowed employees to install metal decking while standing on the outside edge of a steel structural member at a height of approx. twenty-eight (28) feet above a lower level without the use of an appropriate fall protection system.
