29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. On or about February 5, 2013, at the above addressed jobsite, subcontractor employees conducting work on the 2nd floor were exposed to injuries from stepping into holes measuring eleven (11) inches in diameter.
