29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. a. On or about March 26, 2013 at the above addressed jobsite, an employee performing carpentry operations from a scaffold plank which was erected and supported between two tubular welded frame scaffolds, approximately twenty five (25) feet above the ground, was exposed to injuries from falls due to lack of fall protection.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed. a. On or about March 23, 2013 - at the above addressed jobsite, an employee was conducting carpentry operations from an extension ladder that was not used as designed, in that, the employee had to place one foot on a scaffold plank to perform the work. The ladder was placed in a manner that only allowed the employee to have one foot on the ladder rung, thereby exposing the employee to falls of approximately twenty five (25) feet.
