ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1926.405(b)(1): Openings in boxes, cabinets, and fittings through which conductors entered; and unused openings were not effectively closed: a) Northwest end of the building: The wire clamp connecting a 120 volt conductor through the top of the quad receptacle outlet box, did not completely cover the opening, leaving a gap large enough to allow the elements and debris to enter the box. b) Northwest end of the building: A 120 volt quad receptacle outlet had two unused openings on the top of the box covered with common duct tape instead of installing outlet box closure plugs.
