On or about March 21, 2013, at the south side of the clubhouse building, an employee using an air nail gun to install sheathing material was not wearing eye protection, and exposed to being struck by flying debris or nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about March 21, 2013, at the south side of the building, employees installing sheathing on a 6:12 pitch roof were not protected from a 14 feet fall hazard by the use of a fall protection system.
