29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards: a/ At the job site, on or about 03/27/13, specifically at the northend of the intersection of N. 20th Street and Carr Street, insufficient traffic signs were posted at the advanced warning area of the six-day old work zone where employees were installing electric cables in a trench. The trench was located in the southbound lane of N. 20th Street. One orange, diamond-shaped, warning sign that read "Utility Work Ahead" was posted at the intersection of N. 20th Street and Carr Street facing north. A second orange, diamond-shaped, warning sign that read "Right Lane Closed" was posted at the intersection of N. 20th Street and Carr Street facing west. Both lanes on Carr Street at the intersection and to the west were open to traffic.

29 CFR 1926.202: Barricades for protection of employees did not conform to Part VI of the Manual on Uniform Traffic Control Devices (1988 Edition, Revision 3 or Millennium Edition), which are incorporated by reference in 29 CFR 1926.200(g)(2): a/ At the job site, on or about 03/27/13, specifically at the northend of the intersection of N. 20th Street and Carr Street, four, orange, traffic cones were placed in a row at an approximate 45 degree angle from the end of the trench on N. 20th Street where employees were working. The taper ratio for a residential, urban street width would require a minimum of eight traffic cones. The lack of cones provided motorists with little warning to steer clear of the six-day old work zone.

29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA: a/ At the job site, on or about 03/27/13, flaggers or the equivalent were not posted at either end of N. 20th Street where the work zone existed. The work zone existed in the southbound lane of N. 20th Street. Employees were installing electric cables in a trench. Heavy traffic was observed driving in both directions while the work was progressing. INTREN was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.200(g)(2) which was contained in the OSHA Inspection 312726037, citation 1, item 1, that was issued on 07/02/09 and became a final order on 10/23/09.
