29 CFR 1926.404(b)(1)(ii): All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection. Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters. Millinocket Paper Mill - The 6000 watt Lincoln generator, supplying power to the extension cord and DeWalt grinder, was not equipped with a ground-fault circuit interrupter.

In the following instances stairs and passageways were not maintained in a clean safe condition: a) Millinocket Paper Mill - The screening room stairs were blocked by cables. b) Millinocket Paper Mill - The control room passageways contained ice, cables, and debris.

29 CFR 1926.300(c): Employees using hand and power tools and exposed to the hazard of falling, flying, abrasive, and splashing objects, or exposed to harmful dusts, fumes, mists, vapors, or gases shall be provided with the particular personal protective equipment necessary to protect them from the hazard. All personal protective equipment shall meet the requirements and be maintained according to Subparts D and E of this part. Millinocket Paper Mill - The employee using the DeWalt grinder was not wearing eye protection.
