29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a) Roof on the West side of house: On or about March 27, 2013 and at times prior thereto the employer exposed his employees to fall hazards in that he allowed his employees to work on a 5:12 pitch roof without fall protection exposing his employees to falls of 20 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Ladder on North side of house; On or about March 27, 2013 and at times prior thereto the employer exposed his employees to fall hazards in that employees were allowed to access a roof that was 10 feet above the ground with a ladder that was set up where the ladders side rails did not extend at least 3 feet above the upper landing surface. b) Ladder on South side of house; On or about March 27, 2013 and at times prior thereto the employer exposed his employees to fall hazards in that employees were allowed to access a roof that was 10 feet above the ground with a ladder that was set up where the ladders side rails did not extend at least 3 feet above the upper landing surface.
