29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. East side of building - employees were exposed to a 12 ft fall hazard, in that, the use of fall protection was not enforced while working near the edge of the roof. Violation observed on or about 03/26/13.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. East side of building - employees were exposed to a 12 ft fall hazard, in that, the ladder was short and did not extend above the roof landing. Violation observed on or about 03/27/13.
